I understand that The Extension Ballet Studio undertakes to provide students with safe and correct methods of training. TEBS will show all care and due diligence, but can take no responsibility for any injuries that may occur during classes, rehearsals, performances or examinations. No refunds can be given in the event of injury or illness during the class. Instead, students may be allowed to attend a make up class at the studio owner’s discretion. Please note: Ambulance cover is the responsibility of parents or guardians. An ambulance will be called in an emergency, but only after consent from a parent in the case of an injury. I understand that fees must be paid in full by the end of week 2 in each given term. I give permission for images and videos of my child to be used for the purpose of advertising and promoting TEBS.
